Film prikazuje vojno proti mafijskim kartelom z drogami, proti katerim se bojujejo tako na mehiški kakor ameriški strani meje. Na mehiški strani se proti drogam in mamilarskim združbam bori zdravnik dr. Jose Mireles, imenovan tudi kot »El Doctor«, v mestu Michoacan, ki okoli sebe zbira ostale podobno misleče, da bi se s skupnimi močmi uprli opustošenju, ki ga za sabo že dolga leta pušča trgovina z drogami. Na nasprotni strani meje pa podobno paravojaško skupino vodi ameriški veteran Tim »Nailer« Foley, ki želi s tem preprečiti, da bi se mehiške vojne z drogami razširile tudi onkraj mehiške vojne, na ameriško ozemlje. Film nazorno, tudi brutalno, prikazuje krvavo dogajanje »na terenu«, kjer se v boju zoper vojne z drogami hitro zabrisuje črta med dobrim in zlim.
